Athletic Physical Exam Form 2024-25

 

To participate in sports teams at Del Mar, all student-athletes need to download the physical examination form* and submit it online to Athletic Clearance, at www.athleticclearance.com/login.php, or to Mr Castaneda. Please note that as of June 2022, all 9th and 11th grade students must also obtain an electrocardiogram heart screening (ECG). Heart screenings are being offered by the Kyle J. Taylor Foundation.
